By signing below, I acknowledge that my child has permission to participate in the Train:106 Sports Camp and that I have provided accurate information to the best of my knowledge. I understand that by participating in camp activities, my child may be exposed to physical activity that could result in injury. I release Train:106, its staff, and affiliates from any liability related to injuries or accidents that may occur during camp activities. I give my consent for emergency medical treatment.*
